After searching around endlessly for a decent button map template, I gave up and made one in excel and converted it to a pdf form. It's not perfect, but at least it doesn't involve a ton of layers in photoshop and can be printed out on a single piece of paper.

Easy to make, you just need to get some 9mm Pogo pins and wire to extend from end to end
I found the base of the stick too large to fit between legs, this helps with flying and more accuracy for helos etc
